Description
MODIFY AND HOST A WEB-BASED SURVEY THAT IT DEVELOPED FOR A PREVIOUS MINNEAPOLIS RESEARCH GRANT; IT WILL REFINE THE SURVEY BASED UPON THE NEEDS OF THIS CURRENT HSR&D PILOT PROJECT, HOST THE SURVEY, STORE SURVEY RESPOSES AND COMMUNICATE REGULARLY WITH THE MINNEAPOLIS VA CENTER FOR CHRONIC DISEASE OUTCOMES RESEARCH TO PROVIDE INFORMATION ON WHICH RESPONDENTS HAVE/HOAVE NOTE COMPLETED THE SURVEY. PROJECT IMPLICIT WILL BE RESPONSIBLE FOR PROVIDING WAYS TO INCREASE SURVEY RESPONSE RATES TO ACCEPTABLE RATES.
